Welcome to Primary Care 24 (PC24)
Primary Care 24
provides out-of-hours services to Liverpool and surrounding areas
in addition we offer in-hours primary care services.
We are a series of 12 GP practices located in the Sefton and Liverpool area.
Our services run 8am - 6.30 pm, Monday – Friday.
Usual duties on shift include:
• Telephone consultations,
• Face to face consultations,
• e-consultations,
• Directed workflow actions –
from our Primary Care Hub Managers
• Home visits (see below).
COMPLETE ALL YOUR OWN REFERRALS
via the eRS system.
All 2WW and urgent test referral requests must be completed before you finish each shift.
If you are unable to complete a referral for any reason,
you must inform the Hub Manager before your shift ends.
Home Visits
Our home visits requests are usually low,
Within Sefton we can utilise the AVS, “Acute Visiting Service”,
and “Urgent Care Response Team”,
to support patients who are housebound.
Palliative Home visits must be completed by the surgery.
CLINICS
The usual clinics consist of 13 patients in the morning
50/50 split of telephone calls / face to face appointments.
Afternoon clinics consist of ~ 1 5 patients
starting with telephone calls / then face to face appointments.
At the end of each clinic are protected, “GP Bookable Only”, slots.
The hub clinician is on hand to provide support if necessary
and in he event of any unforeseen emergencies.
Our PRIMARY CARE HUB
based in Litherland supports all 12 practices
through the provision of a centralised administrative function.
The Hub also has GPs and ANPs supporting online consultations,
managing onward referrals,
reviewing clinical investigation reports,
and managing / coding medical documents.
Our Clinical Pharmacy Team
manages the majority of prescription requests and queries.
PRACTICE HUB sessions
:
As part our new way of working, some shifts are 10.5 hours.
The day consists of 8 hours clinical practice as outlined for the Primary Care Hub Clinicians above
and 2.5 hours of directed administrative duties.
If you are undertaking one of these shifts, one of our Hub Managers
will contact you during the day to advise you of the work allocated to you.
An appointment book
will be allocated to your session to include the following duties:
• Online Health Forms
• Telephone Triage
• Urgent home visits (ie, EoL)
• Directed workflow actions – directed from our Primary Care Hub Managers
• Admin actions ie, referral rejections
You are expected to action the above by contacting the patient directly (text or telephone)
DO NOT SEND TASKS for further action.
Urgent requirements – you must contact the hub manager on duty.
SUPPORT RECEPTION
In addition, we ask that you support our reception staff when on shift
with any additional clinical queries that occur throughout the day.
